Vaccinations against Cervical Cancer
Cervical cancer and its prevention: what you should know
Cervical cancer is the second most common cancer worldwide in women under 45. If you have ever had a sexual relationship you may be at risk of cervical cancer - whatever your age or family history, and regardless of the number of partners you have had.
The good news is that great progress is being made in the prevention, diagnosis and treatment of cervical cancer. But do you know what actually causes cervical cancer and what you can do to protect yourself? Read on to learn more about this important issue and feel free to contact us to talk through any questions you may have about cervical cancer or its prevention.
What is the cervix?
The cervix protects your uterus by acting as a barrier to infections. It is the lower, narrow end of the uterus (womb) that forms a canal between the uterus and vagina. The cervix allows the menstrual blood and also a baby to pass from the uterus into the vagina, and sperm to pass from the vagina into the uterus.
What causes cervical cancer?
You may be surprised to learn that the main cause of cervical cancer is a virus. Scientists have proven that virtually all cervical cancer is caused by certain types of a virus called human papillomavirus (HPV).
How is HPV spread?
HPV is spread through sex and even sometimes during intimate genital skin-to-skin contact. Therefore, every woman who has ever been sexually active, regardless of age, is at risk of catching it. In fact, three quarters of sexually active women will be infected with HPV at some time in their lives.
Condoms are very effective against sexually transmitted diseases. However, they do not offer full protection against HPV, as the skin immediately around the genital area can also carry the virus.
How does cervical cancer develop?
HPV is very widespread and easily transmitted. Fortunately your immune system will usually be able to fight off an HPV infection without you ever knowing you have been infected. However if the virus remains in the cervix, abnormal cells can develop. In such cases these abnormal cells, if not identified early, can develop over a period of years into cervical cancer.
The importance of cervical screening
In the UK, cervical screening has been hugely successful in reducing the incidence of cervical cancer. Whilst cervical screening does not prevent infection with HPV, regular testing is the most effective way of detecting virus-related cervical cell abnormalities, which, if left untreated, can develop into cervical cancer.
Many lives have been saved as a direct result of smear tests. So, whatever you do, remember to attend for regular cervical screening. Do call us to when your next test is due.
What does cervical screening involve?
During the test your doctor will use an instrument called a speculum to gently hold the vagina open to allow them to see your cervix more clearly, They will then use a special brush device to gently collect a few cells from the surface of your cervix. These sample cells will be sent away to a laboratory to be examined for possible abnormalities. Should any abnormal cells be found, you will be contacted and advised about any further tests or treatments,
Screening only takes about 5 minutes and most women agree that, although perhaps a bit uncomfortable or undignified, they do not hurt and are over very quickly.
Should you be worried about an abnormal test result?
It is important to remember that an abnormal test result does not necessarily mean that cervical cancer has been found or will develop, The laboratory has simply discovered some changes in the cells that need further investigation, Many such changes will disappear of their own accord, However; in some instances these abnormalities have the potential to develop into cervical cancer, which is why further investigation is required,

Can HPV infection be prevented?
Cervical screening is tremendously important in detecting cervical cell changes. You are therefore strongly encouraged to attend regular cervical screening. Cervical screening does not, however, prevent infection with HPV.
Vaccinations are also now available that provide immunity against some of the major cancer-causing strains of HPV, and may be something that you might like to discuss with the doctor.
Cervical cancer vaccination
Vaccination can give you protection against the two most common cancer-causing types of HPI/. This helps prevent the majority of cervical cancers.

Costs of Vaccination
The Practice now has availability of two vaccines that can help to prevent cervical cancer. One of the vaccines is manufactured by GlaxoSmithKline, the other is manufactured by Sanofi Pasteur, part of Merck. Both vaccines work by developing antibodies to HPV. The cost per course of 3 vaccines is £450 for the vaccine manufactured by Sanofi Pasteur, and £480 for the vaccine manufactured by GlaxoSmithKline. This is inclusive of the initial consent consultation of £65. Free registration with the practice is included.

PANDEMIC FLU
Pandemic Flu is Flu that spreads rapidly causing widespread epidemics around the world. Pandemic flu occurs when a new, highly infectious and dangerous strain of the influenza virus appears. In contrast to the 'ordinary' or 'seasonal', flu outbreaks which we see every winter in the UK, flu pandemics occur infrequently- usually every few decades. There were three last century. The most serious was in 1918, killing millions of people worldwide. Smaller pandemics happened in 1957 and 1968.
There remains a possibility that Avian Flu could mutate into a Pandemic Flu although the likelihood of this is unknown.
Many individuals and businesses have decided to prepare for a worst case scenario by putting in place a plan to help protect their families and staff in a worst case event.
The Government has put in place a plan to distribute antiviral drugs to 25% of the population in the event of an outbreak of Pandemic Flu. However it is not yet clear how these antivirals will be distributed and to whom.
If an outbreak of Pandemic Flu were to reach the UK it is estimated that it would take between 4-6 months before an effective vaccine could be developed.
The best known anti viral drug at present is a prescription only drug manufactured by Roche Pharmaceuticals called Tamiflu®.
Private General Practice Services has met with Roche to discuss an appropriate programme for the distribution and effective use of Tamiflu®, and is an authorised supplier of Tamiflu®.
Private General Practice Services will only distribute Tamiflu® to those individuals and businesses which agree to abide by the guidelines for distribution and effective use of Tamiflu®.

Anti Viral treatment against Pandemic Flu
Gold Service
A telephone call from Dr Piper's Practice will take place to discuss processes and procedures. We will then write to all recipients instructing them to read through the Tamiflu® instructions and use as appropriate (either for short-term prevention ie a 10-day course, a six week prevention for key senior personnel and possibly treatment if a person has symptoms).
For those key personnel requiring a six-week course of treatment:

IF AND WHEN A PANDEMIC DOES KICK IN, THERE WILL BE AN ADDITIONAL CONSULTATION FEE FOR THOSE PATIENTS WHO REQUIRE IT.
Silver Service

Bronze Service
As above but issue of drugs is reserved until there is an outbreak of Tamiflu® and meantime boxes of Tamiflu® are stored either at PGPS or our nominated storage company, Mawdsley (www.mawdsleys.co.uk/contactus.asp). Contact name is Daren Belcher.
Cost of Tamiflu® to purchase is £35 per head (per box) plus storage costs (check with Daren Belcher). At the time of a pandemic, PGPS will organise a nurse to contact a key person in the company and register the employees, also organise distribution of the medications.
Cost is £50 per head for distribution and consultation by phone.
Paediatric Service
As for adult except for the 30 mg capsules which are a lower price.Paediatric 30 mg capsule prices are Gold Service £180, Silver Service £70, Bronze £30 plus storage costs; with issue price £50 (as for adults).
