DR JULIA PIPER, BM, BS, BMedSci, MRCGP, DFFP, DipOccMed, DipAccMed

Dr Julia Piper founded Private General Practice Services in 1995. She is a member of the British Medical Association, the General Medical Council, the Medical Protection Society, the British Society of Medical and Dental Hypnosis, the Royal College of General Practitioners, the British Medical Acupuncture Society, the Faculty of Family Planning and Reproductive Health, The Royal Society of Occupational Medicine and the Independent Doctors' Forum. Dr Piper was previously Regional Physician in Charge for Bupa Health Screening and BUPA Wellness in Leicester, Birmingham and Lincoln. She worked with BUPA for almost twenty years, being involved with setting some of their national protocols. She has also been involved with medical journalism, medical teaching and assisting local NHS Practices.

Julia sits on the council of the Independent Doctors Federation in London previously having been responsible for IT within the independent sector of the UK and currently responsible for continuing professional development within the independent sector.

Julia believes strongly in integrated medicine, the power of traditional medicine coupled with alternative treatments such as acupuncture, hypnotherapy, massage, physiotherapy and reiki. Julia has a Diploma in Accupuncture and is recognised by most insurers for Acupunccture treatment. She also practises Hypnotherapy and self Hypnotherapy. Julia is also leading the way with Functional Medicine, which is an understanding of the biochemistry of the body, and orthomolecular medicine which combines an understanding of the body’s biochemistry with extremely high doses of vitamin treatment. In particular she has a strong understanding of the orthomolecular treatment of schizophrenia, following the work of Abram Hoffer and maintaining the excellence of this wonderful treatment since his death earlier this year (March 2009) at the age of 91. Abram Hoffer was the father of functional and nutritional medicine and spent his whole life developing this subject at times in the face of great adversity. His dedication and prowess in the field of Orthomolecular Medicine, together with his work with Linus Pauling, (Double Noble Peace Prize Winner) has set the stage for much of Dr Piper’s current work. DR JEREMY FENBY-TAYLOR    BSc, MB, BS, MRCGP, DOccMed

Jeremy is a colleague to Dr Piper in the private sector as well as a part time NHS General Practitioner in Oakham. He is a member of the British Medical Association, General Medical Council, Royal College of General Practitioners, Royal Society of Occupational Medicine and Medical Defence Union. He has many years experience of providing Occupational Health services to UK companies in a wide variety of sectors. He ofers both Private GP consultations and Occupational Health consultations including Sickness Absence Medicals, Pre-employment Medicals, Health Checks etc. He is able to undertake joint injections for shoulders, elbows etc

DR JOE CHAN    MB, ChB, FRCSE, DFFP, MRCGP, DipMedAcc.

Dr Joe Chan is a colleague to Dr Piper in the private sector as well as being a partner in an NHS practice in Leicester. He consults at the practice on Monday afternoons. He provides Private GP Consultations and has a strong interest in Functional Medicine and Nutrition. He also has a diploma in Acupuncture and is able to undertake joint injections for shoulders, elbows etc.

DR NISHA KARIA, B.Sc. (Hons.), PsychD., C.Psychol.

Dr. Nisha Karia is a Clinical Psychologist chartered with the British Psychological society (BPS). Dr. Karia qualified as a Practitioner Doctor of Clinical Psychology (PsychD.) in 2004 after 9 years of academic, research and clinical training from the University of Surrey via BPS recognised and accredited courses. Through this training, further professional development courses and relevant pre- and post-qualification clinical practice, Dr. Karia has developed her knowledge, skills and experiences in working psychologically with people across the lifespan and with a variety of developmental, clinical and mental health needs.

Consultancy, Teaching and Training, Supervision

Dr. Karia is able to provide Consultancy Services and also teaching and training on mental health issues and cases. She has extensive experience of delivering training to a variety of audiences, and will tailor each training package to suit the target audience. Most recently, she has delivered training on ‘Acculturation and mental health in Young Asian Adolescents’. Dr. Karia can also provide clinical supervision to other health professionals.

Psychological Assessment

When a referral is accepted, an Appointment is usually offered for some form of psychological assessment in the first instance. Assessments are carried out for various reasons. Typically, a clinical assessment interview will be Conducted to get a general overview of the problem, the client, and the contexts and systems within which they live. It is also used to ascertain how the problem developed in the person’s life and what type of Intervention, if any, is needed. Assessment is an on-going process even when therapy begins as new information often comes up and changes happen over time.

Other methods of psychological Assessment include observation, getting reports from other people/professionals involved in the client’s life (with the client’s permission) and psychometric assessment. Psychometric assessment involves the use of tests or rating scales to investigate and measure traits, moods, symptoms, intellect, Behaviours, and emotions, to name a few. Psychometric measures are Standardised, which allows comparisons to be made with the client’s report and what would be a typical response from similar others. Cognitive psychometric assessment is useful for measuring functioning of the brain. So, for example, this would involve using a psychometric test to assess for memory, attention, concentration, and learning difficulties.

Psychological Intervention

Dr. Karia uses a number of Theoretical and therapeutic perspectives to guide her interventions, including cognitive behavioural therapy, systemic and narrative therapy approaches, solution-focussed ideas, psychodynamic ideas, developmental models, and models of psychological coaching. This allows the intervention to be tailored Specifically to the client and their needs. The information acquired in the assessment prior to intervention, is used to develop an agreed psychological ‘formulation’, which is a working Hypothesis about the origin of the problem, how it developed, what maintains it, and what would be the most helpful way to proceed.

Intervention usually involves some Variation on ‘talking therapy’ depending on the specific model of therapy used. Clients are often asked to carry out tasks in-between sessions to continue the therapeutic effect and consolidate what was discussed in the Session.

Structure of Appointments

Typically, clinical psychology Sessions last for fifty minutes and start and end promptly. Assessment sessions are likely to last a little longer, though session duration can be negotiated on an individual basis. Sessions are held in Consulting Rooms, though it is sometimes necessary to do home visits and conduct treatment sessions outside of a clinical consulting room setting.

Quality Control and Ethics

Dr. Karia had to pass stringent Membership Requirements in order to achieve her Chartered status and practising certificate. As a Chartered psychologist, she must adhere strictly to the BPS code of conduct and ethics. Furthermore, the BPS require Dr. Karia to demonstrate evidence of Continual Professional Development and Supervision to annually renew the Practising Certificate.

Confidentiality

Most of what is discussed in Sessions is kept confidential between Dr. Karia and the client. However, Dr. Karia will need to report back to the referrer about the outcome of the assessment/intervention, though precise details of what is included in those Reports can be negotiated. If it would help the assessment or intervention to request information from others, whether they are professionals or significant others, Dr. Karia would usually ask permission or consent from the client. The only exception to this is if it is felt that there are serious issues of risk involved to the client or to others by not sharing information, in which case Confidentiality will be breached.

Working Hours

Due to her full-time NHS commitment, Dr. Karia is only able to offer appointments outside of normal working hours (Early Mornings, Evenings after 5.00p.m., and weekends)

Measles In Europe

As we write, there have been over 1000 cases of Measles in France. Switzerland and Germany also have significant problems with measles. This is because these countries do not have such good immunisation coverage as we do in the UK. By contrast there have been 80 cases in England. The most recent outbreak of measles is at Leeds University where so far 15 students have been affected. The first case was exposed during a skiing trip to France.
